Abstract Although conventional tyrosine kinase inhibitor have revolutionized the management of KIT and PDGFRA mutant gastrointestinal stromal tumor (GIST), the subset of patients with PDGFRA D842V mutation lags behind, with poor survival outcomes. The only drug approved in this setting, avapritinib, is inaccessible and difficult to afford in many parts of the world and can cause considerable toxicity. Therefore, it is the need of the hour to expand the therapeutic armamentarium for this patient cohort. We report the case of a 48-year-old female patient with localized PDGFRA D842V-mutant gastric GIST who developed metastatic disease progression while on adjuvant imatinib. She was started on cabozantinib, to which she had a partial radiologic response. This is the first case report demonstrating the potential utility of the drug in this challenging subset of patients with advanced GIST.
