A Longitudinal Study of Very Low‐birthweight Infants. IV: An Overview of Performance at Eight Years of Age

SUMMARY A cohort of 169 very low‐birth weight infants (800 to 1500g) was followed prospectively to the age of eight years. Information on 159 of the children was available at that age, and for a further five to the age of six years. A comparison group of 67 normal‐birthweight children was also followed, but only 43 of these children were seen at the age of eight years. Eight of the very low‐birthweight (VLBW) children were attending special schools and 156 attended normal schools: placement of the remaining five was unknown. Of the VLBW children who were adequately assessed, 15‐9 per cent were either not reading or were retarded by more than 18 months. Mean VLBW full‐scale IQ was 88‐8, compared with 98 ‐8 for the normal‐birthweight group. A subgroup of 10 VLBW children with birthweights under lOOOg had lower mean scores on all three WISC‐R scales compared with those with birthweights between 1000 and 1500g. Of the adequately assessed VLBW children, 3–7 per cent had epilepsy, 3–7 per cent had significant sensorineural deafness and 2–4 per cent suffered from cerebral palsy. One or more visual defects were detected in 31 ‐4 per cent of the children, though in only 3–8 per cent was the defect serious. At eight years, growth dimensions for the VLBW children were below the 10th percentile for 11? 0 per cent in weight, 16 1 per cent in height and 15‐0 per cent in head circumference. Degrees of handicap in the VLBW children were profound in 51 percent, severe in 10‐8 per cent and signifcant in 40‐5 per cent: handicap was minimal or absent in the remaining 43‐6 per cent. For the normal‐birthweight group the percentages were 2–3, 0, 25‐6 and 72‐1 respectively. The VLBW children with minimal or no handicap were considered to have a satisfactory outcome. This was significantly correlated with the following: gestation over 30 weeks; birthweight over 1199g; no major apnoeic attacks; lowest postnatal weight before six days; return to birth weight before 21 days; peak serum bilirubin not exceeding 255/umol/litre; and parental social‐class between 2 and 5 on the seven‐point Congalton Scale. Huit des enfants de tres faible poids de naissance (TFPN) fréquentaient des écoles spéciales et 156 des ecoles normales: le placement des cinq restants est inconnu. Parmi les enfants TFPN correctement examinés, 15, 9 pour cent ne lisaient pas ou présentaient un retard de plus de 18 mois. Le Ql moyen à ' échelle globale pour le groupe TFPN etait de 88, 8 compare au 98, 8 pour le groupe de poids de naissance normal. Cette evolution favorable etait significativement corrélée avec les facteurs suivants: grossesse de plus de 30 semaines, poids de naissance de plus de 1199g, pas de crise majeure d'apnee, la pointe de chute de poids post‐natale avant six jours; le retour de poids de naissance avant 21 jours; la pointe de bilirubine serique restant endessous de 225;μmol/litre et la classe sociale des parents entre les scores 2 et 5 a la Congalton Scale a 7 niveaux. Von 159 der Kinder liegen Informationen bis zu diesem Alter vor und fur weitere fiinf bis zum Alter von sechs Jahren. Eine Vergleichsgruppe von 67 Kindern mit normalen Geburtsgewicht wurde gleichzeitig nachuntersucht, jedoch konnten nur 43 dieser Kinder bis zum Alter von acht Jahren gesehen werden. Acht der Kinder mit sehr niedrigem Geburtsgewicht (SNGG) besuchten Spezialschulen und 156 gingen zu normalen Schulen: der Verbleib der restlichen fiinf war unbekannt. Von den genau untersuchten Kindern mit SNGG hatten 3, 7 Prozent Epilepsie, 3, 7 eine signifikante sensoneurale Taubheit und 2, 4 Prozent eine Cerebralparese. Eine oder mehr Sehstorungen wurden bei 31, 4 Prozent der Kinder entdeckt, jedoch nur bei 3, 8 Prozent handelte es sich um eine ernsthafte Stoning. Im Alter von acht Jahren lagen fiir die Kinder mit SNGG folgende Wachstumsparameter unterhalbder 10ten Percentile: für 11, 0 Prozent im Gewicht, 16, 1 Prozent in der GroBe und 15 Prozent beim Kopfumfang. Der Grad der Behinderung bei den Kindern mit SNGG war bei 5, 1 Prozent hochgradig, bei 10, 8 Prozent schwer und bei 40, 5 Prozent signifikant: bei den restlichen 43, 6 Prozent war die Behinderung minimal oder nicht vorhanden. Fiir die Gruppe der Kinder mit normalen Geburtsgewicht lagen die Prozentsatze in gleicher Reihenfolge bei 2, 3, 0, 25, 6 und 72, 1. Die Kinder mit SNGG, die minimal oder gar nicht behindert waren, hatten ein zufriedenstellendes Ergebnis. IV: Resumen de su realization a los ocho anos Un grupo de 169 lactantes con muy bajo peso al nacer (800 a 1500g) fueron seguidos prospectivamente hasta la edad de ocho anos. La information en 159 de estos ninos se consiguio hasta la edad de ocho años y para los otros cinco casos hasta la edad de seis anos. Un grupo comparativo de 67 ninos con peso de nacimiento normal fue tambien seguido pero solamente 43 de estos ninos fueron vistos a la edad de ocho anos. Ocho de los ninos con muy bajo peso al nacer (MBPN) acudian a escuelas especiales y 156 acudian a escuelas normales: el emplazamiento de los otros cinco era desconocido. De los ninos MBPN que fueron evaluados adecuadamente, 15, 9 por ciento no sabian leer o sufrian un retraso de mas de 18 meses.
