Introduction and Objective: Polycystic Ovarian Syndrome (PCOS) affects up to 12% of women of reproductive age and is associated with increased risk of insulin resistance and impaired glucose tolerance. This study evaluated baseline HbA1c levels and longitudinal HbA1c trajectories in adolescents with PCOS to identify characteristics associated with elevated or increasing HbA1c and to inform opportunities for enhanced screening and support. Methods: This retrospective cohort study used electronic health records from a free-standing pediatric hospital (January 2016-July 2025). Patients with a confirmed diagnosis of PCOS were included. Longitudinal HbA1c trajectories were evaluated using linear mixed-effects models with random intercepts and slopes, examining the impact of key demographic and clinical characteristics. Results: A total of 819 patients met inclusion criteria, with 0-7.2 years of follow-up. Mean HbA1c (5.4) across the population remained stable, but several factors were associated with increased HbA1c throughout the study period, including higher BMI (β=0.06, 95% CI=0.03, 0.09), older age at PCOS diagnosis (β=0.03, 95% CI=0.00, 0.05), Asian or unknown/declined race (β=0.18, 95% CI=0.05, 0.31 and β=0.25, 95% CI=0.04, 0.46, respectively) and acanthosis (β=0.18, 95% CI = 0.07, 0.28). Ethnicity moderated trajectories: HbA1c declined over time in non-Hispanic patients and increased in Hispanic patients (β=0.12, 95% CI=0.05, 0.20). Language also moderated patterns, with those with a language of care (LOC) of Spanish showing decreasing HbA1c relative to English and other LOC (β=-0.13, 95% CI=-0.23, -0.02). Conclusion: Early PCOS identification may help prevent rising HbA1c in higher-risk groups, including adolescents identifying as Asian or unknown/declined race. Patients from some groups may benefit from continued support after diagnosis. Language patterns suggest that culturally and linguistically aligned care may be protective for some patients.
