Motivation: To investigate the brain change of TLE patients. We aim to further search the key brain regions that influence seizure. Goal(s): dentify the brain subregions most affected by TLE or those that have the greatest impact on the development of TLE. Approach: The brain was segmented into subregions by using automated technology. Comparison between groups was used to identify differential brain aeras and we applied mediation analysis to explore the relationships between regions. Results: In TLE patients, certain brain regions showed volume differences that are associated with cognitive changes and disease duration. Additionally, the thalamus may be a key subregion influencing cognitive changes. Impact: Our study found an association between the thalamus and cognition, providing a new approach for understanding the mechanisms of epilepsy and developing targeted treatment strategies.
