Persistent Mullerian duct syndrome (PDMS) is characterized by the presence of Mullerian duct derivatives in a phenotypically and genetically normal individual. This is a case of a male in his 40s who was being evaluated for infertility. Ultrasonography revealed pelvic structures resembling ovaries and a uterus. After an adnexectomy, histology showed Mullerian duct derivatives (fallopian tube) and seminoma in an atrophic testis.
