Abstract Benign bone cysts of the proximal femur are a challenge to treat. There are a plethora of treatment options ranging from careful neglect, single or multiple intralesional steroid injections, curettage with or without adjuvant, and bone grafting. We report a case of an adolescent girl who presented with a large simple cyst of the proximal femur, and it was successfully treated with curettage and bone grafting, along with screw augmentation. The child gained full function without any complication in spite of the very large size of the cysts. Extended curettage, bone grafting, along with trans-physeal screw augmentation, can lead to healing and full and uneventful recovery for large cystic lesions of the femur if carefully planned.
