TÓM TẮT Hội chứng Raynaud là một rối loạn co thắt mạch ngoại vi, dẫn đến thiếu máu cục bộ tạm thời. Khi tình trạng kéo dài, thiếu máu mạn tính có thể gây loét, hoại tử da đầu chi. Nghiên cứu trường hợp này báo cáo một trường hợp bệnh nhân mắc hội chứng Raynaud có hoại tử da đầu chi được điều trị thành công bằng phương pháp kết hợp giữa Y học hiện đại (YHHĐ) và Y học cổ truyền (YHCT). Bệnh nhân nam 61 tuổi, tiền sử hút thuốc lá nhiều năm, nhập viện vì đau và tím đen đầu ngón tay, ngón chân. Khám lâm sàng và siêu âm Doppler xác định hội chứng Raynaud kèm tắc hoàn toàn động mạch chày sau trái. Phác đồ điều trị bao gồm thuốc giãn mạch, thuốc kháng kết tập tiểu cầu, kiểm soát yếu tố nguy cơ theo YHHĐ, kết hợp bài thuốc Dương Hòa Thang gia giảm và liệu pháp ôn điện châm theo YHCT. Sau 10 tuần điều trị các triệu chứng hết hoàn toàn. Không ghi nhận tác dụng không mong muốn. Từ trường hợp bệnh trên cho thấy hướng tiếp cận đều trị kết hợp YHHĐ và YHCT có thể giúp cải thiện triệu chứng và chất lượng cuộc sống cho bệnh nhân hội chứng Raynaud. Từ khóa: Hội chứng Raynaud, Dương Hòa Thang, ôn điện châm, tắc động mạch chày sau, Y học cổ truyền. SUMMARY Raynaud’s syndrome is a peripheral vasospastic disorder that leads to transient ischemia. When prolonged, chronic ischemia may result in digital ulcers and skin necrosis of the extremities. This case report presents a patient with Raynaud’s syndrome complicated by digital necrosis who was successfully treated using a combined approach of modern medicine and traditional medicine. The patient was a 61-year-old male with a long history of cigarette smoking, admitted to hospital due to pain and cyanotic-black discoloration of the fingertips and toes. Clinical examination and Doppler ultrasound confirmed Raynaud’s syndrome accompanied by complete occlusion of the left posterior tibial artery. The treatment regimen consisted of vasodilators, antiplatelet agents, and risk factor control according to modern medicine, combined with a modified Duong Hoa Thang herbal formula and warm electroacupuncture therapy based on traditional medicine principles. After 10 weeks of treatment, all clinical symptoms resolved completely. No adverse effects were recorded. This case suggests that an integrative treatment approach combining modern medicine and traditional medicine may effectively improve symptoms and quality of life in patients with Raynaud’s syndrome.
