Case: A 16-year-old boy with habitual patellar dislocation during knee extension accompanied by bilateral severe trochlear dysplasia and excessive lateral deviation of the tibial tubercle underwent medial patellofemoral ligament reconstruction with switching wedged tibial tubercle transfer osteotomy. This technique involves making 2 wedge-shaped osteotomies on the tibial tuberosity and its medial side in the axial plane and subsequently inverting the 2 bone fragments to move the tibial tuberosity medially. Conclusion: Switching wedged tibial tubercle transfer osteotomy combined with medial patellofemoral ligament reconstruction can help enable substantial correction in cases of severe tibial tubercle lateralization.
