Background: Massive cardiomegaly is a potentially life-threatening consequence of a range of primary and secondary cardiovascular pathologies, from hypertrophic cardiomyopathy to severe longstanding hypertension. 1 Obesity alone may precipitate massive cardiomegaly in the absence of underlying cardiac pathology. 2 In the post-mortem setting, cardiomegaly presents an especially complex diagnostic challenge with significant ramifications for first degree relatives of the deceased.
