This comment argues for a critical update to the diagnostic criteria for nail matrix melanoma. Based on recent evidence of cases in younger patients, we propose replacing the ‘adult age’ criterion in the ABCD rule with ‘after puberty’ to facilitate earlier detection. The shift underscores the importance of dermoscopic vigilance and timely biopsy in adolescents and young adults with atypical longitudinal melanonychia.
