Herlyn-Werner-Wunderlich syndrome, also referred to as obstructed hemivagina and ipsilateral renal anomaly (OHVIRA) syndrome, is an uncommon congenital malformation involving the Mllerian ducts.It typically presents with a duplicated uterus (uterus didelphys), a blocked hemivagina on one side, and the absence of the kidney on the same side.Symptoms typically appear after menarche and often include pelvic pain or the presence of a pelvic mass.In some cases, patients may present with primary infertility, although this is less common.We report three cases of OHVIRA syndrome diagnosed over a 3-year span.Diagnosis was established through magnetic resonance imaging (MRI) of the abdomen and pelvis, along with confirmation by laparoscopy.All patients underwent surgical treatment involving vaginal excision of the obstructing vaginal septum.Given the rarity of this condition, maintaining a high index of clinical suspicion is essential for timely identification and acting promptly for the best outcomes, avoiding complications, and maintaining fertility.
