Management of lipid profile and lipid-lowering therapy in the diabetic population of a primary care center according to cardiovascular risk category (SCORE2-Diabetes vs. SCORE)

Introdução e Objetivos: As doenças cardiovasculares estão entre as principais causas de morbimortalidade e acarretam custos significativos, sendo a Diabetes Mellitus tipo 2 (DM2) um fator de risco major. Após se verificar que uma elevada proporção de doentes com DM2 apresentava valores de LDL-C acima do recomendado, decidiu-se avaliar os níveis de LDL-C em função do valor-alvo estabelecido para cada categoria de risco cardiovascular (RCV), assim como a terapêutica hipolipemiante prescrita. Métodos: Trata-se de um estudo transversal com uma amostra aleatorizada de 245 utentes, entre os 40 e os 69 anos, com registos recentes de perfil lipídico e função renal. Foram definidos como critérios de exclusão: doença macrovascular, insuficiência renal ou doença microvascular em múltiplos territórios. Resultados: Verificou-se que 59.2% dos utentes apresentavam LDL-C acima do valor-alvo, dos quais mais de 75% eram de alto ou muito alto risco. Entre estes, 27,6% não estavam sob qualquer terapêutica hipolipemiante e em apenas 9% estavam prescritas estatinas de alta intensidade, isoladamente ou em associação com ezetimiba. Nos doentes medicados com estatina de alta intensidade e ezetimiba, 78,9% atingiram o valor-alvo de LDL-C. A categoria de RCV foi superior no SCORE face ao SCORE2-Diabetes em 56,7% dos casos, influenciando significativamente a proporção de utentes com LDL-C no alvo (20,0% vs. 40,8%, respetivamente; p=0,014). Conclusões: A terapêutica hipolipemiante continua subutilizada em doentes com DM2, particularmente nos de maior risco cardiovascular, já que mais de metade não atinge o valor de LDL-C alvo. Surpreendentemente, o modelo SCORE parece sobrestimar o risco cardiovascular em comparação com o SCORE2-Diabetes. Introduction and Objectives: Cardiovascular diseases are among the leading causes of morbidity and mortality and represent a significant economic burden; having type 2 diabetes is a major risk factor. After identifying that a considerable proportion of patients with type 2 diabetes were not achieving their recommended LDL-C targets, a study was designed to assess LDL-C levels in relation to target values defined by cardiovascular risk (CVR) category and the lipid-lowering therapy (LLT) prescribed. Methods: This cross-sectional study included a random sample of 245 patients with diabetes, aged 40 to 69 years, who had recent lipid profile and renal function tests and were being followed at a primary care (PC) center. Exclusion criteria were established for macrovascular disease, significant renal impairment, or microvascular complications in multiple territories. Results: It was observed that 59.2% of patients had LDL-C levels above target, of whom over 75% were classified as high or very high risk. Among these, 27.6% were not receiving any LLT, and only 9% were being treated with high-intensity statins, either alone or in combination with ezetimibe. Among patients receiving this combination, 78.9% achieved target LDL-C levels. CVR category was higher in the SCORE model than the SCORE2-Diabetes in 56.7% of cases, significantly influencing the proportion of patients with LDL-C at target (20.0% vs 40.8%, respectively; p=0.014). Conclusions: Lipid-lowering therapy remains underutilized in patients with type 2 diabetesTYPE 2 DIABETES, particularly those at high and very high CVR, as over half fail to achieve LDL-C targets. Notably, the SCORE model appears to overestimate CVR compared with SCORE2-Diabetes.
