Implementing routine annual COVID-19 vaccination for adults aged 65 years and older in South Korea represents a highly value-based strategy that significantly reduces mortality and severe health outcomes compared with no vaccination. The vaccination program remains economically attractive even under conservative scenarios, such as lower disease incidence or shorter duration of vaccine protection, confirming its robustness as a long-term public health investment. These findings support a policy transition from temporary emergency responses to a sustainable National Immunization Program, ensuring stable funding and consistent protection for vulnerable older populations in the post-pandemic era.
