Introduction: Dialysis adequacy is an important factor that affects the outcome of CKD patients on dialysis.Kt/V determination is a way of assessing dialysis adequacy.Daugirdas formula and online clearance monitoring (OCM) are methods that can be used to determine Kt/V.This study aimed to determine the Kt/V using Daugirdas Formula and Online Clearance Monitoring and to compare the Kt/V values using these methods Methods: Kt/V of 54 CKD patients on regular hemodialysis (HD) were determined using Daugirdas formula and OCM in 4 treatments.Pre and post HD BUN, duration of dialysis, ultrafiltration volume and post dialysis weight were used to compute for Daugirdas Kt/V.OCM Kt/V was taken from the HD machine.Paired T-test, Wilcoxon signed rank test and Pearson correlation were used to compare, analyze and determine the association of OCM and Daugirdas Kt/V.Results: Mean Kt/V from Daugirdas is 1.3982, which is significantly higher than that of OCM which is 1.1127.Mean difference was 0.2855.The t-value of 8.804 at 53 degrees of freedom yields a p-value of <0.001, indicating that this difference is highly significant.The computed effect size of 1.204 is considered very large, signifying a substantial magnitude of discrepancy between the two measurement methods.With Wilcoxon signed rank test, the test statistic of 1437.0 with a standardized z-value of 5.980 and a standard error of 0.155 yielded a p-value < 0.001, indicating a highly significant difference between the two paired measures.The effect size of 0.935 is considered very large, emphasizing that the discrepancy between Daugirdas and OCM measurements is statistically significant.The Pearson correlation analysis between the average Daugirdas and OCM Kt/V revealed a strong, positive and statistically significant relationship with an r = 0.634 and a p value < 0.001.Conclusion: There is significant difference between Kt/V values using OCM and Daugirdas.While OCM may be useful for monitoring relative changes, reliance solely on it may underestimate dialysis adequacy compared to the established Daugirdas method.I have no potential conflict of interest to disclose.I did not use generative AI and AI-assisted technologies in the writing process.
