Abstract

Abstract Background Since 2003, the Orthopaedic Data Evaluation Panel (ODEP) has benchmarked orthopaedic implants using revision rates at specific time points, progressively tightening these standards to match global improvements. ODEP assigns A*, A, and B ratings based on evidence strength. This study evaluates revision rate variation among Total Hip Replacement (THR), Total Knee Replacement (TKR), and Total Shoulder Replacement (TSR) implants submitted for ODEP benchmarking since 2021. The goals were to analyse variation, suggest updated survival thresholds, and assess registry-related differences. Methods Revision data for 364 THR, 534 TKR, and 128 TSR implants from 2021 onward were analysed. Revision rates were plotted against benchmarks at 3, 5, 7, 10, 13, and 15 years. Registries were noted, and implant data anonymised. Results Revision rates increased over time across all joints, with notable variation between implants at each benchmark. Registry source sometimes influenced rates. Some implants maintained revision rates comparable to early benchmarks (3 years) even at 15 years. Proposed reductions in revision thresholds would reclassify several implants, mainly affecting THR after 7 years and TKR from year 5, especially at 13 and 15 years. Conclusions Lowering revision thresholds for A and A* ratings would substantially impact implant classifications, supporting updates to survival benchmarks to reflect current implant performance.
