An 11-year-old girl presented with 9 months of left wrist pain, worse at night and partially responsive to NSAIDs. Imaging showed nonspecific bone marrow edema on MRI, but CT revealed a small nidus in the triquetrum. Surgical excision confirmed osteoid osteoma, with complete symptom resolution. Osteoid osteoma of the triquetrum, though rare, should be considered in children with chronic wrist pain. CT is essential for diagnosis, and surgical excision leads to excellent outcomes.
