Irreparable posterosuperior rotator cuff tears with severe infraspinatus fatty infiltration present a complex biomechanical challenge. Comparison between superior capsular reconstruction and combined superior capsular reconstruction with lower trapezius transfer suggests potential advantages of a dual approach. In this commentary, this commentary discusses whether both procedures are truly necessary or whether LTT alone may account for most of the observed benefits. Furthermore, it highlights the need for future comparative studies including isolated LTT to clarify the real contribution of each technique.
