Introduction: Bronchiectasis is a chronic respiratory disease characterized by impaired mucociliary clearance, recurrent infections and persistent sputum production, often leading to reduced quality of life. Airway clearance techniques are a cornerstone of respiratory rehabilitation, particularly in the home setting. Intrapulmonary oscillation devices, such as Simeox®, have emerged as innovative tools to enhance mucus mobilization, though real-world evidence in domiciliary contexts Clinical Case Description: We report the case of an adult patient diagnosed with bronchiectasis, followed in a home-based respiratory rehabilitation program. The intervention consisted of the use of an intrapulmonary oscillation device (Simeox®) over a period of three months, integrated into a structured domiciliary care plan. The patient received education and training aimed at self-management, correct device use, and adherence to airway clearance strategies. Clinical follow-up focused on respiratory symptoms, sputum clearance, treatment adherence and perceived impact on daily functioning. Over the intervention period, the patient demonstrated improved effectiveness of sputum expectoration, reduction in respiratory symptoms, and increased confidence in self-management of the disease. Adherence to the therapeutic plan was high, supported by caregiver/patient education and regular monitoring. Conclusions: This case highlights the potential benefits of incorporating intrapulmonary oscillation therapy with Simeox® into home-based respiratory rehabilitation for patients with bronchiectasis. The combination of an innovative airway clearance device and structured patient education may enhance symptom control, promote autonomy and support long-term disease management in the domiciliary setting. Further studies are warranted to strengthen evidence on its clinical impact.
