Abstract We describe a case with sagittal craniosynostosis accompanied by a midline cephalohematoma. Preoperative computed tomography (CT) revealed fusion of the center of the sagittal suture, under the cephalohematoma. Postoperative microCT of en bloc specimen confirmed a fused suture along cephalohematoma, with thicker more mature outer layer of bone compared to the inner layer. Histological analysis revealed suture present in both superior and inferior bone layers surrounding the hematoma. We conclude craniosynostosis gave rise to the midline cephalohematoma.
