Laparoscopic inguinal hernia repair is widely adopted due to its minimally invasive nature and faster postoperative recovery. However, rare mesh-related complications such as appendicitis and appendicocutaneous fistula have been described. Case Presentation: We present the case of a 63-year-old male with chronic groin pain and sinus discharge two years post-TAPP. Imaging was inconclusive, and diagnostic laparoscopy revealed the appendix entrapped within the mesh. Appendectomy and anterior fistulectomy were performed without mesh removal, resulting in full recovery. Discussion: This report adds to the scarce literature describing mesh-induced appendiceal involvement, with discussion of pathophysiology and comparison to previously reported cases. Conclusion: Although extremely rare, awareness of this complication is critical. Adequate peritoneal coverage during TAPP can prevent visceral entrapment.
