This case highlights that right coronary artery intervention might trigger painful left bundle branch block syndrome, which can be successfully managed with oral beta-blockers.
We present a rare case involving the diagnosis and treatment of coronary atherosclerotic heart disease complicated by painful left bundle branch block. In July 2023, an elderly Asian male patient was admitted to the Aerospace Center Hospital with recurrent angina as the main symptom. A retrospective analysis was carried out. Coronary angiography showed severe stenosis in the right coronary artery. After its interventional treatment, the patient's chest pain changed. Through dynamic electrocardiogram monitoring and treadmill exercise testing, he was diagnosed with Painful Left Bundle Branch Block Syndrome. After taking oral beta - blockers, his symptoms improved during follow - up. Usually, this syndrome occurs in those without coronary artery disease and can be missed in patients with coronary heart disease. This case shows that right coronary artery intervention might trigger this syndrome. We hope this case can offer new ideas for future relevant clinical work.
