We present a case of a 20-year-old male who developed unilateral optic neuritis 2 months after receiving the yellow fever 17DD vaccine. He reported decreased visual acuity and eye movement pain in the left eye. Examination revealed a relative afferent pupillary defect, red desaturation, and optic disc edema. Infectious, autoimmune, and demyelinating causes were excluded. MRI of the brain and orbits was unremarkable. He was treated with intravenous corticosteroids followed by oral taper, achieving full recovery without recurrence after 10 months. Although causality remains uncertain, this case suggests a possible delayed immune-mediated reaction following yellow fever vaccination. Prompt diagnosis is critical.
