This study analyzed the burden of age-related macular degeneration (AMD) in China from 1990 to 2023 using data from the Global Burden of Disease 2023 (GBD 2023), focusing on temporal trends, gender- and age-specific disparities, and the contribution of smoking to AMD burden. In 2023, China recorded 2,258,548 prevalent cases and 156,474 years lived with disability (YLD) due to AMD; the age-standardized rate (ASR) per 100,000 population was 99.70 for prevalence and 6.92 for YLD, representing a 19.92% and a 38.76% reduction from 1990, respectively. The ASR of prevalence and YLD of AMD showed a statistically significant decreasing trend (average annual percent change (AAPC) < 0, P < 0.05). Females carried a significantly higher burden than males, particularly those aged ≥ 45 years, with the highest burden observed in the ≥ 95 years group. The proportion of AMD-related YLD attributable to smoking in China was 13.59%. Gender- and age-targeted prevention strategies and strengthened tobacco control are imperative to alleviate the AMD burden. • The burden of AMD in China showed statistically significant decreasing trends during 1990-2023. • Marked gender- and age-specific disparities existed in China’s AMD burden. • Smoking contributes 13.59% of AMD-related YLD in China.
