ABSTRACT Background: Follicular unit excision (FUE) is a cornerstone technique in modern hair restoration, traditionally requiring shaving of the donor area. Non-shaven follicular unit excision (NS-FUE) has emerged as an alternative approach that preserves donor hair length, offering enhanced discretion and immediate post-operative cosmetic acceptability. Methods: This article presents technical refinements of the NS-FUE technique based on the author’s clinical experience, including detailed approaches to anesthesia, donor harvesting, graft handling, and implantation. The integration of contemporary motorized FUE systems and trumpet-style punches is discussed, with practical comparisons to conventional shaved FUE and long hair FUE (LH-FUE). Clinical Observations: In the author’s experience, NS-FUE appears to demonstrate low transection rates and preservation of high-quality follicular unit grafts. The technique enables maintenance of the patient’s natural hair appearance throughout the peri-operative period, with reliable graft survival and favorable aesthetic outcomes. Conclusion: NS-FUE represents a technically advanced refinement of conventional FUE that prioritizes patient discretion without compromising graft quality. Although modern instrumentation enhances efficiency and precision, optimal outcomes remain highly dependent on surgical expertise, careful patient selection, and meticulous technique.
