A heart-transplant recipient developed fatal neurologic disease. High-throughput sequencing (HTS) identified Dobrava - Belgrade virus in central nervous system (CNS) and peripheral specimens. Genomic analyzes showed distinct viral populations across compartments, suggesting CNS tropism. This case highlights atypical infection presentations in immunosuppressed patients and underscores the diagnostic value of HTS.
