Necrotizing fasciitis (NF) is a life-threatening soft tissue infection characterized by rapid progression and high morbidity and mortality rates. This historical review examines the evolution of management strategies and clinical outcomes of necrotizing fasciitis since 1990, highlighting significant advancements in diagnosis, treatment, and patient care. The pathophysiology of NF involves a complex interaction between bacterial virulence factors, host immune responses, and local tissue environments, which has been increasingly understood through ongoing research. Since the early 1990s, the identification of causative organisms has expanded beyond the traditionally implicated Group A Streptococcus and Staphylococcus aureus to include a broader spectrum of pathogens, including polymicrobial flora involving anaerobes and Gram-negative bacteria. This shift has prompted changes in empirical antibiotic therapy, with current guidelines advocating for broad-spectrum coverage that is tailored to the specific microbiological profile of the infection. Surgical intervention remains the cornerstone of management, with early and aggressive debridement shown to significantly improve outcomes. The review discusses the evolution of surgical techniques, emphasizing the importance of timely intervention. In recent years, the integration of advanced imaging modalities and the use of adjunctive therapies—such as hyperbaric oxygen therapy and intravenous immunoglobulin—have provided additional tools in the management of NF, although their roles remain under continuous investigation. Outcomes of necrotizing fasciitis have improved over the decades, attributed to enhanced awareness, early diagnosis, and the implementation of multidisciplinary approaches involving surgeons, infectious disease specialists, and critical care teams. The review also addresses the psychosocial impact of NF on survivors, including long-term physical and emotional sequelae, underscoring the need for holistic patient care. This historical analysis provides a comprehensive overview of the progression in the management of necrotizing fasciitis since 1990, identifying trends, challenges, and future directions in research and clinical practice.
