Abstract Introduction HealthWISE was jointly launched by ILO and WHO as a technical tool to improve the occupational health of health workers (HWs) in 2014. The application of HealthWISE in China has been advocated since 2016. During the COVID-19 pandemic, the ninth HealthWISE training-of-trainers (TOT) was organized in 2021. The study aims to evaluate the effectiveness of HealthWISE TOT and the practice in the pilot hospital that hosted the TOT. Methods The TOT was evaluated among participants using mixed methods research, combining two-run surveys with a questionnaire between pre-training and six-month post-training, qualitative interviews, and on-site observations on the practice of the pilot hospital. Results Among 49 participants, the questionnaire survey showed that the HealthWISE implementation score increased significantly from “111.76±1.61” in pre-training to “124.79±1.66” in post-training (P 0.01). The benefits of training from 49 interviewees were summarized under four themes of occupational safety and health (OSH): enriching professional knowledge, changing beliefs and attitudes, preparing for action, and providing good training models. The observations suggested both good and poor practices in the pilot hospital, focusing on musculoskeletal disorders, biological hazards, and workplace violence. Discussion The ninth national HealthWISE TOT highlighted the participatory characteristics, with learning-by-doing practice, which led to a positive effect among participants. The pilot hospital has improved the occupational health of HWs. Further actions need to focus on workplace violence and the OSH management system. Conclusion The TOT is necessary for the sustainable implementation of HealthWISE, which has a positive impact on improving the OSH management system for HWs.
