Arterial ischemic stroke (AIS) is a rare but devastating condition. Endovascular mechanical thrombectomy (EMT) is the standard of care in adult AIS. There is evolving evidence for the use of EMT for large vessel occlusion in pediatric AIS. Challenges in pediatric stroke are adherence to the suggested timeline for intervention, expertise availability, and the size of the vessel. We report a 3-year-old child with AIS (posterior cerebral artery thrombosis), who underwent EMT and had a good outcome. A multidisciplinary approach, meticulous neuro-intensive care, and intensive neuro-rehabilitation help achieve good functional outcomes.
