We report India’s first documented case of a fourth renal transplant in a 33-year-old male with recurrent Immunoglobulin A (IgA) nephropathy. Surgery was complicated by dense adhesions, vascular atherosclerosis, and postoperative hypotension. He later developed MI and gastrointestinal perforation requiring laparotomy and right hemicolectomy. Despite challenges, the patient recovered well with stable graft function, highlighting the feasibility of repeat transplants.
