Primary central nervous system T-cell lymphoma (PCNSTL) is a relatively rare type of extranodal non-Hodgkin lymphoma. It is extremely rare in patients with primary spinal cord involvement. We present 18 F-FDG PET/CT findings of primary central nervous system T-cell lymphoma of the spinal cord in a 46-year-old man. 18 F-FDG PET/CT showed multiple FDG-avid foci in the spinal cord and right adrenal gland. Pathological examination after T12 vertebral spinal cord surgery confirmed the diagnosis of primary central nervous system T-cell lymphoma.
