Permanent His bundle pacing may be an effective treatment for the disabling symptoms of painful left bundle branch block syndrome.
Chest pain associated with the development of left bundle branch block (LBBB) that is unrelated to coronary ischemia has been recognized as “painful LBBB syndrome.”1,2 Specific LBBB patterns associated with this syndrome have been identified,2 but its true prevalence in the general population is unknown owing to coexisting coronary artery disease (CAD) and lack of widespread awareness of such a clinical entity. This syndrome can cause disabling symptoms with significant activity limitation. The proposed mechanisms include dyssynchronous systolic motion of the septum and generalized interoceptive sensitivity.
