ABSTRACT Introduction: Mucoid degeneration of the anterior cruciate ligament (ACL) is an uncommon knee condition caused by the deposition of mucinous material within the ACL in the absence of a synovial lining. Magnetic resonance imaging is specific for the diagnosis of mucoid ACL. The characteristic “Celery stalk” appearance reflects ACL degeneration. Treatment includes arthroscopic debridement, notchplasty, and ACL resection. Materials and Methods: This prospective study (January 2023–May 2025) included eight patients (five males and three females) presenting with knee pain without instability. After diagnostic arthroscopy, debulking was performed using a 4.0-mm shaver through the anteromedial portal. Results: This study included five males and three females with a median age of 39.5 years (range: 29–47 years). Partial debulking was performed in seven patients, while one patient underwent complete debulking with concomitant ACL reconstruction. The mean follow-up period was 5.5 months (range: 3–8 months). At the final follow-up, all patients achieved a good range of motion with no clinical instability. Conclusion: Arthroscopic debulking of the mucoid degeneration of the ACL is the treatment of choice. The choice of ACL reconstruction should be considered for young patients and those with high demands. A longer follow-up is still required to rule out residual anterior laxity, which we lack in our study.
