Chronic hand eczema can cause major functional and occupational impairment and may be difficult to manage when systemic therapies are contraindicated. We report real-world use of topical delgocitinib in severe, treatment-refractory disease, demonstrating rapid clinical improvement, return to work, and reduced monitoring burden compared with systemic therapy. This case series experience supports topical delgocitinib as an effective and practical therapeutic option in selected patients.
