Introduction and Objective: Blue Circle Health (BCH) was created with support from the Helmsley Charitable Trust to explore whether an innovative care model can achieve improved outcomes at reduced costs for people with type 1 diabetes. BCH provides a 6-month virtual care, education and support program, intended to augment patients’ usual care. This study aimed to assess changes in hemoglobin A1c (HbA1c) pre- and post-participation. Methods: We enrolled 259 patients across 15 Federally Qualified Health Centers in Florida between Jan 9, 2024 and Mar 17, 2025. Pre HbA1c was the closest prior value within 180 days of enrollment; post HbA1c was the closest subsequent value within 180 days of the final encounter. Lab or point of care values were accepted. We analyzed outcomes for participants who completed the 6-month program (“Full Circle”), those who exited early (“Early Exit”), and the combined sample. Results: A total of 92 participants with eligible HbA1c data were included. The mean age was 38 years; 57% identified as female, 28% as Caucasian, 40% as Hispanic, and 30% as Black; 26% were uninsured, 33% had government insurance, and 50% were unemployed or on disability. Among Full Circle participants, mean HbA1c decreased by 0.86% (p = 0.004). Early Exit participants showed a mean decrease of 0.42% (p = 0.047). Across all participants, mean HbA1c decreased by 0.63% (p 0.001). Conclusion: Findings demonstrate statistically significant reduction in HbA1c after participation in BCH. Disclosure M.
