Abstract Histoplasmosis is a chronic granulomatous fungal infection caused by the dimorphic fungus Histoplasma capsulatum . Usually it is prevalent in immunocompromised patients and is endemic to South America, Africa, and the central and eastern part of the United States; however, India is considered as nonendemic zone for it. Here, we report such a case of disseminated histoplasmosis in a 54-year-old male presented with generalized symptoms like fatigue, dyspepsia, and weight loss. Computed tomography (CT) scan of the abdomen with an adrenal protocol revealed bilateral, bulky adrenal glands with heterogeneous enhancement and cystic areas. Histopathology of a CT-guided biopsy from the right adrenal gland, along with special stain examination, confirmed the diagnosis of adrenal histoplasmosis. The patient was treated with itraconazole and supportive therapy. On 6 month of follow-up, he was found to be asymptomatic and doing well.
