Motivation: 2D Flip Angle Modulated (FAM) CSE-MRI is a novel, free-breathing fat and iron quantitation sequence that still needs to be validated in the clinical setting. Goal(s): To compare the IQ and quantitative metrics of free-breathing 2D FAM CSE-MRI and breath-held 3D CSE-MRI in the clinical setting at a single site. Approach: 79 patients were scanned with both 2D FAM and 3D CSE-MRI sequences. A trained radiologist assessed the IQ/apparent SNR/artifacts/PDFF/R2* values for each of the two sequences. Results: 2D FAM CSE-MRI showed improved IQ, apparent SNR, and fewer artifacts compared to the 3D CSE-MRI, while showing no statistically significant difference in PDFF/R2* values. Impact: With its free-breathing nature and reproducible PDFF and R2* values compared to gold standard 3D CSE-MRI, 2D FAM CSE-MRI shows promise as a routine tool in clinical liver fat and iron quantitation.
