For this reason, we aimed to investigate the impact of BMI on perioperative morbidity in patients diagnosed with a renal mass and surgically treated. METHODS: Within a prospectively maintained database, patients with cM0 renal masses who underwent surgery at a single tertiary center were identified. Intraoperative (IntraOp) and Post-procedural (PostOp) complications were prospectively collected. PostOp were categorized according to the Clavien-Dindo (CD) classification. Logistic regression models tested the association between BMI and the occurrence of intraoperative and postoperative complications, adjusting for age, comorbidities, cT stage and surgery type (partial vs radical nephrectomy). RESULTS: Among 3364 patients presenting with cM0 renal tumors, 12% (n=402), 13% (n=445), 75% (n=2517) were treated with a ROB, LAP and OP approach, respectively. In patients treated with ROB, 5% (n=20) and 36% (n=146) reported IntraOp and any PostOp complications (CD>0), respectively and BMI was associated with an increased risk of IntraOp complications (Odds Ratio (OR): 1.02; 1.01-1.03; p=0.02) but not of PostOp complications (OR: 1.00; 0.89-1.03; p=0.2). In patients treated with LAPs, 4% (n=18) and 17% (n=77) reported IntraOp and PostOp complications, respectively and BMI was not associated with an increased risk of IntraOp complications (OR: 1.03; 0.93-1.15; p=0.6) or PostOp complications (OR: 1.01; 0.92-1.03; p=0.4). In patients treated with OP, 9% (n=214) and 27% (n=671) reported IntraOp and PostOp complications, respectively and BMI was not associated with an increased risk of IntraOp complications (OR: 1.01; 0.99-1.02; p=0.1) or PostOp complications (OR: 0.99; 0.98-1.01; p=0.7) (Figure 1). CONCLUSIONS: Higher BMI is associated with more intraoperative but not postoperative complications during robotic renal surgery. These findings can improve preoperative discussions, enabling tailored care plans such as preoperative weight loss strategies, if suitable, for patients.
