To introduce a novel therapeutic approach for managing posttraumatic iris cysts. A retrospective review was carried out on five patients who underwent surgical intervention for posttraumatic iris cysts. In case 1, complete surgical excision was performed, whereas a more conservative combined surgical approach was adopted for cases 2 through 5. Preoperative and postoperative evaluations included visual acuity, intraocular pressure, slit-lamp examination, and ultrasound biomicroscopy findings. In case 1, the patient developed significant postoperative complications, including a corneal fistula. A modified conservative combination surgery was employed in subsequent cases, resulting in favorable anatomical and functional outcomes. This case series offers a new and efficient method to treat posttraumatic iris cysts.
