Introduction: Endometrial carcinoma with predominant squamous differentiation is rare and diagnostically challenging. Pilomatrix-like high-grade endometrioid carcinoma (PiMHEC) is a recently recognised aggressive variant, while primary squamous cell carcinoma (PSCC) of the endometrium is also exceedingly uncommon. We present two challenging cases and compare their clinicopathologic and immunohistochemical features.
