ABSTRACT Biologic corneal inlays can rarely present with immediate postoperative decentration, which can be subtle and difficult to detect clinically. Real‐time tomography allows precise early diagnosis, while same‐day flap relifting and careful repositioning provide a safe, effective strategy to restore centration and preserve visual quality.
