Ethiopia has implemented government health insurance programmes to extend healthcare services to rural areas, aiming to improve access and utilization among underserved populations. An action research approach was employed, involving surveys in selected rural communities and semi-structured interviews with healthcare providers. Data were analysed using descriptive statistics and thematic analysis. Survey results indicated that only 35% of surveyed households had enrolled in the insurance programme despite its free coverage for essential health services. Despite initial coverage expansion, challenges such as low awareness and insufficient outreach efforts hinder effective utilization of the programmes. Immediate measures should include targeted public education campaigns and enhanced community engagement strategies to boost enrollment and usage rates.
