Adenomatoid odontogenic tumor is an extremely rare tumor of odontogenic epithelial tissue origin, mostly reported in the maxilla, rarely in the mandible. The extrafollicular presentation of adenomatoid odontogenic tumor in the lower anterior mandibular region in a middle-aged female patient presented as a localized gingival enlargement within the oral cavity for the last 6 months. The growth was excised locally, and thorough curettage was done; the histopathological report revealed the lesion to be an extrafollicular variant of adenomatoid odontogenic tumor. This rare representation of adenomatoid odontogenic tumor in the mandibular jaw and its management were described along with clinical and radiological features in this article.
