The IOM informs federal government, but is independent. A wide-ranging review of US emergency care provision was recently performed by the great and the good of US emergency medicine. This was mainly in response to emergency department overcrowding (no 4-h target!), diversity of prehospital care provision (for example a third of emergency medical systems (EMS) providers in the USA are volunteers) and a suspicion that paediatric emergency care (PEC) was not as good as it could be.
