A 6-year-old girl with an acral pigmented lesion initially diagnosed as a nevus was referred for surgical excision. Dermoscopy raised suspicion of tinea nigra, which was confirmed by mycological examination identifying Hortaea werneckii, and the lesion resolved with topical antifungal therapy.
