Abstract Pertinent issues surrounding the utilisation of naturalistic communication therapy with adult aphasics are presented. Discussion centres around the need to place greater emphasis in management programmes on aphasics using language in everyday contexts, for different purposes. The relevance of discourse analysis is considered and procedures for the assessment and treatment of aphasic communication are outlined. The need to measure the effectiveness of such therapy is highlighted.
