In a 71-year-old woman with non-small cell lung cancer and a rare coronary artery to bronchial artery anastomosis, surgical resection was successfully performed despite intraoperative bleeding.

This case demonstrates that lung resection can be safely performed without inducing myocardial ischemia in a patient with a coronary-bronchial artery anastomosis, despite intraoperative challenges in bleeding control.
症例は73歳女性.66歳時に狭心症疑いにて当院循環器内科で冠動脈造影を施行.その際に冠動脈気管支動脈吻合症を指摘された.72歳時の胸部CTで右上葉S2に7 mm大の部分充実結節を指摘され,経時的に増大するため診断と治療を兼ねて手術を施行する方針となった.冠動脈造影およびCT angiographyでは,右冠動脈と気管支動脈系との吻合を確認した.手術は鏡視下に開始し,腫瘍を部分切除し術中迅速組織診にて腺癌を確認したため右S2区域切除の方針とした.肺門部周囲には異常に発達した気管支動脈網を認め,これら異常血管からの出血コントロールに難渋したため開胸に移行して最終的に右上葉切除を施行した.気管支動脈を処理することで冠動脈への血流の影響が懸念されたが,術中・術後を通して心筋虚血を疑う心電図変化は認めなかった.周術期管理の反省点および文献的考察を加えて報告する.
