Conversation-based digital therapeutics (CBDTx) integrate behavioural science, dialogue systems, and personalised care to deliver adaptive, context-aware health interventions. To support their efficient and scalable development, model-driven approaches are increasingly employed, aiming to streamline implementation, enable adaptation across diverse clinical scenarios, and improve software quality. However, the conceptual and technical foundations of such systems remain fragmented, as existing studies often address isolated aspects of adaptation, messaging, or context modelling. This paper presents a systematic literature review of studies that conceptually represent and model aspects of CBDTx, focusing on conceptual aims, modelling approaches, and the degree of integration. The review highlights key thematic areas, identifies current gaps, and explores how conceptual modeling approaches can provide a unified framework to guide the design and deployment of future CBDTx systems.
