The report highlights the occurrence of Wenckebach block in the distal conduction system in patients with trifascicular disease presenting with syncope or dizziness.
Summary: Two patients presented with dizziness or syncope and electrocardiographic evidence of intraventricular and/or atrioventricular conduction abnormalities. Both had prolonged HV intervals at rest indicating trifascicular disease. Wenckebach block in the bundle branch system was present in one patient at rest and in the other during atrial pacing. The significance of these findings are discussed.
