These patients constitute a small percentage of individuals with heart disease; but this is not a rare clinical picture, as is quite evident in view of the recent literature. Over the years there has been little added to the understanding of the basic derangement that results in heart failure in these patients. It would seem that the defect lies within the myocardium.
