In this paper we use the case of the London Ambulance Service Computer Aided Despatch (LASCAD) project as a cogent example of the nature of information systems failure. Our aims are twofold: to set the LASCAD case in the context of previous case studies of IS failure and to use a web approach to organize the case material and hence assess the importance of two important explanatory frameworks for considering IS failure.
